To see patients on Fay, you must maintain active credentials. Specifically, all providers are required to have:
An active CDR license
Active Professional Liability Insurance (PLI)
If either of these credentials expires or becomes invalid, you will receive a notification from Fay. While your credential is inactive, you may be unable to see patients until it has been updated and approved.

Once submitted, our team will review your documentation. Verification timelines can vary, and you'll receive an email once your credential has been reviewed and approved.

If your Professional Liability Insurance is rejected or you're having trouble uploading it, check the information below.
My PLI coverage doesn't meet the required limits
Check your Certificate of Insurance (COI) to confirm your coverage amounts.
Fay requires minimum coverage of:
$1,000,000 per occurrence
$3,000,000 aggregate
If your coverage is below these limits, contact your insurance provider to increase your coverage.
My insurance is Commercial Liability, not Professional Liability
Fay requires Professional Liability Insurance (PLI) (also called Malpractice Insurance). Commercial General Liability (CGL) or other general business insurance does not satisfy this requirement. Check your COI to confirm it specifically lists Professional Liability coverage. If it doesn't, contact your insurer to obtain the correct policy.
My PLI policy isn't active yet or has already expired
Your insurance policy must be active today.
If your policy starts in the future, contact your insurer to move the effective date sooner.
If your policy has expired, you'll need to renew your coverage before uploading a new COI.
My PLI policy is expiring soon
You'll need a renewal policy that provides uninterrupted coverage after your current policy expires. If you don't yet have a renewal policy, contact your insurer to renew or purchase continued coverage.
My PLI doesn't list me as individually insured
Your Certificate of Insurance must list your name as the Owner or Named Insured. If only your LLC or practice name appears on the policy, ask your insurer to provide an addendum that includes your name as an individually insured provider.
My PLI document is being rejected
Before uploading, make sure you're submitting:
A Certificate of Insurance (COI) (not a declarations page)
A document that clearly shows Professional Liability/Malpractice Insurance and your full name as the Owner or Named insured
A minimum coverage of: $1,000,000 per occurrence and $3,000,000 aggregate
A document with "Certificate of Insurance" displayed at the top of the page
A current active PLI
Documents that only show Commercial General Liability or declarations pages cannot be accepted.
