What is CAQH ProView, and why does it matter?
CAQH ProView is a secure, centralized online database where healthcare providers enter and maintain their professional and practice information. As a provider on Fay, you are responsible for keeping your CAQH profile up to date and re-attesting every 120 days, as required by insurance companies. Re-attestation is essential to maintain your active credentialing status and remain in-network with insurance plans.
Note: Once your profile is successfully re-attested, you will see two checkmarks on your CAQH profile.
Where can I access my CAQH profile?
Your CAQH ProView profile is accessible at proview.caqh.org. You should have created an account during your onboarding with Fay.
Note: If you have forgotten your password, click Forgot Password on the login page to reset it. If you did not create a CAQH account during onboarding, contact [email protected] with the subject line "Assistance Needed for CAQH Login Information."
How do I re-attest my CAQH profile?
To re-attest your CAQH profile:
Log in to your CAQH profile and click the Review & Attest button on the right-hand side. This will display any outdated information or errors that need to be corrected
Review and update your account information as needed
Click Review & Attest again to complete your re-attestation
Important: If you have added a new state license or insurance policy, upload the corresponding document in the Documents section before submitting.
How do I update my professional liability insurance (PLI) in CAQH?
To add new PLI information after your previous policy has expired:
Navigate to the Professional Liability Insurance section under Profile Data
Click + Add under Insurance Coverage
Enter your new policy information. Unless your specific policy states otherwise, use the following responses:
Unlimited coverage with this insurance carrier: No
Individual coverage: Yes
Self-insured: No
Click Review & Attest and go to the Documents section to upload your new policy
Note: The status of your uploaded document will change from Received to Approved a few days after you have successfully uploaded your policy.
How do I update my state license information in CAQH?
To update an expired state license in CAQH:
Navigate to the Professional IDs section under Profile Data
Click Edit for the license state you would like to update
Enter your current license information and confirm the following:
License number is correctly entered. For states where RD licensure is not yet required or operational (currently AZ, CA, CO, VA, MI, NJ), enter your CDR number as your license number
Currently Practicing is set to Yes
License Status is set to Active
License Type is not mandatory, but based on your state license type (e.g., LN), you can enter your License Type
Issue date and expiration date are both filled in
If License Unlimited? is set to No, provide the additional information as prompted
Ensure all of your state licenses have been added to Professional IDs. If you see placeholders for states that require state licensure where you are not licensed (i.e., states other than CA, AZ, MI, NJ, VA), delete those state license entries.
Note: MI and NJ have passed licensure laws but have not yet finalized regulations — this status may change, and Fay will notify you if it does.
I am seeing additional state license and practice location information I do not recognize. Should I remove it?
No, do not remove Fay-associated entries from your CAQH profile. Practice states in your CAQH profile reflect every state where you are authorized to see patients via telehealth. Some states (such as NJ, CA, AZ, CO, VA) are included because licensure is not currently required there, meaning you can lawfully see patients in those states without a separate state license. The practice location address in those states reflects Fay's credentialing address, which is required by insurers for in-network participation.
For Fay practice locations, select Confirm to complete the attestation.
Note: Feel free to add additional information to your profile. Additional forms and attestations may be required after new information is added to your profile.
I am seeing additional blank forms after re-attesting. What should I do?
Additional attestation and disclosure forms may be required depending on your practice states. Download the relevant forms, sign and date them, and upload the completed documents to finish your re-attestation.
What should I check for when re-attesting my CAQH profile?
Before completing your re-attestation, confirm the following:
Licensure and certification
All active and inactive state licenses are listed with issue and expiration dates
For states without a license requirement (e.g., CA, NJ, AZ), your CDR number is listed as the state license number
Your active Commission on Dietetic Registration (CDR) number is listed under Specialty
Your NPI (Type 1, individual) is correctly listed
Education
Bachelor's degree, highest degree, and internship relevant to your provider type are listed
Institution names match your diploma
Professional liability insurance
Active personal PLI policy is uploaded, listing your full name as the insured
If your LLC is listed as the insured, ensure your full name and Fay Nutrition are both listed as Additionally Insured
Policy covers telehealth services and is not limited to in-person care
Coverage meets $1M per incident / $3M aggregate
Employment history
Fay Nutrition is listed as your current employer
At least 5 years of employment history is included, with explanations for gaps over 6 months
Disclosures and documents
Disclosure questions are answered accurately, with explanations for any "Yes" responses
All required documents are uploaded
CAQH and any state-specific attestations are signed and dated
State-specific applications and documents are uploaded where required
