You can search your inbox for "Typeform" to locate where you confirmed acceptance of the Provider Terms of Service, and you can search your inbox for "Docusign" to see the additional documents you signed. The Provider Terms of Service Agreement is also available and contains the same information.
What happens to my patients if I discontinue my practice on Fay?
If you discontinue your practice on Fay, patient continuity is handled as follows, in accordance with Section 8 of the Provider Terms of Service Agreement:
Fay-sourced patients: Fay will proactively reach out to Fay-sourced patients to ensure continuity of care and assist them in transitioning to another provider on the platform
Self-sourced patients: continuity planning for self-sourced patients is at your discretion
