Referrals and HMO plans

Some insurance plans, particularly HMO plans, require a referral before nutrition services can be covered. Documentation may also be needed for certain diagnoses or to help a patient get the most coverage available under their plan.

Where can I check if a patient needs a referral?

Open the patient in the Patient tab and look under Patient details. The referral field may say:

  • Required: The patient’s plan or billing diagnosis requires a referral. Visits won’t be covered until one is validated.

  • Recommended: A referral isn’t required, but documentation of a qualifying condition may help the patient get the most coverage available under their plan.

  • Not required: No referral is needed for the patient’s current coverage.

If a referral is required or recommended, you may also see its status:

  • Not submitted: No referral is on file.

  • Awaiting validation: A document has been submitted and is being reviewed.

  • Validated: The submitted referral has been accepted.

  • Rejected: The submitted document could not be validated. The patient will need to submit new documentation.

Select View to review a referral on file. A submitted referral does not meet the requirement until it has been validated.

What referral documentation can a patient submit?

The patient should upload one of the following:

  • A referral confirming their qualifying medical condition

  • A note from a recent visit confirming their qualifying medical condition

The document may be signed by a physician or an advanced practice provider (APP), such as a nurse practitioner (NP) or physician assistant (PA). Make sure it clearly includes:

  • The patient’s qualifying medical condition

  • The medical provider’s name

  • The medical provider’s signature

  • The date the document was signed

How does a patient submit a referral?

Ask the patient to:

  1. Log in to their Fay account using a web browser.

  2. Go to Settings.

  3. Select Docs & Forms.

  4. Upload their referral documentation.

Patients can upload images, PDFs, and Word documents up to 10 MB. Fay’s billing team will review the submission and contact the patient if any additional information is needed.

Can I upload a referral for a patient?

Yes. If you already have a patient’s referral, you can upload it on their behalf:

  1. Go to the Patient tab, open the patient, and go to Documents.

  2. Select Add document, choose Physician referral, and select the file.

You can upload images, PDFs, and Word documents up to 10 MB. Fay’s billing team will review the submission and contact the patient if anything else is needed.

Where can I see a patient’s uploaded documents?

You can view documents uploaded to Docs & Forms from the Patient tab under Documents. Uploaded files may include:

  • Physician referrals

  • Supporting medical documentation

  • Insurance cards

  • Other coverage-related documents

Check the referral field in Patient details to see whether a referral is required and whether a submitted referral has been validated.
