How does Fay handle patient intake forms?
Fay automatically sends intake forms to new patients on your behalf. The intake forms are designed using best practices, so no setup is required on your end.
How do I access a patient's completed intake form?
To access a completed patient intake form in Fay, there are two ways:
Select Clients from your sidebar menu and select the patient you are looking for. Scroll to the bottom of the Client Details and select View intake questions to review the patient's answers
Click on your patient's appointment from your Calendar and select View Profile
Can I edit the intake form sent by Fay?
Intake forms sent by Fay cannot currently be edited. If you would like to send additional forms to your patients, you can share them directly via Fay Chat.

What if my patient has not completed their intake form before the appointment?
If a patient has not completed their intake form ahead of the appointment, contact them directly by phone or text using the phone number listed on their profile. Reaching out by phone or text is recommended over Fay Chat, as some patients may not have logged into Fay yet and may not see your message.
You can also send your patient this link to complete their intake form ahead of the appointment. If a patient is unable to complete their intake form before the appointment, walking through intake questions together during the initial consultation is a good way to gather the background information you need and ask any follow-up questions.
Release of information (ROI) form
How do I access my patient's release of information (ROI) form?
To access a completed release of information form, select Clients from your sidebar menu, select the patient you are looking for, and under Client Details, select Download release form.
How can my patient make changes to their release of information (ROI) form or add members to their care team?
If a patient needs to update their release of information (ROI) form or make changes to their care team, they can do so in two ways:
Update their release of information form directly using this link
Add, change, or remove members of their care team through the Fay app by navigating to Profile and selecting Care team
